Injury Scheme Forms

  • Male Members (Adult & Juvenile)

In the event of an Adult or Juvenile male player sustaining an injury the following procedures must be adhered to:

  • Inform the referee of your injury and give the referee your name.  If the injury only becomes apparent after the match then inform your manager immediately who must contact the referee with your name and details of the injury in order to have it submitted with the referees report.  If the injury is sustained at training inform your manager.  The manager will then inform the club secretary –
  • All claims are required to be reported by the club to the administrators Willis Towers Watson within 60 days of the injury. Failure to comply with this will render the claim null and void.  In order to meet the 60 day requirement, all injuries should be notified to the club secretary/club injury administrator as soon as possible by completing the attached Injury Claim Notification Form.
  • A player must attend a doctor or dentist in order to validate an injury claim - please refer to the attached GAA Injury Benefit Fund Summary Document for full details, exceptions and limitations.  The attached Medical Certification will need to be completed by the attending doctor for submission with the claim.
